Nexa Speech Concierge is a clinician-owned speech therapy practice providing skilled, in-home adult and geriatric speech therapy services across Florida’s Treasure Coast, Palm Beach region, and surrounding communities.

We serve adults with dysphagia, cognitive-communication disorders, aphasia, dysarthria, voice concerns, neurogenic communication disorders, dementia-related communication needs, and other medically based speech therapy needs.

Our care is provided in the setting where function matters most: the patient’s home.

Position Overview
Nexa Speech Concierge is seeking a motivated Speech-Language Pathologist to provide in-home adult medical speech therapy services on a flexible, contract basis. Nexa is currently prioritizing clinicians available to serve patients in Palm Beach County, with additional opportunities across the Treasure Coast and surrounding South Florida communities.

This role is ideal for an SLP who enjoys adult medical care, values autonomy, and wants the flexibility of in-home practice without feeling clinically isolated.

Unlike many traditional settings, clinicians at Nexa are not simply handed a large caseload across a broad region. Because every patient has a zip code, we build caseloads thoughtfully based on geography, clinician availability, bandwidth, clinical strengths, and patient need.

This is a community-based/in-home role. The listed address is used for job-board location purposes only and is not a brick-and-mortar reporting site.

Clinical Growth & FEES Opportunities
For clinicians interested in dysphagia and advanced medical practice, Nexa is building pathways for growth.

Within our concierge model, clinicians may have opportunities for exposure to advanced dysphagia management, medically complex adult care, interdisciplinary communication, and FEES-related learning as the company continues to grow.

Prior FEES experience is not required, but interest in dysphagia, instrumental swallowing assessment, and adult medical practice is highly valued.

This is a strong fit for an SLP who wants to move beyond generalist therapy and grow toward more specialized adult medical care in a flexible, in-home setting.

Primary Responsibilities:
Responsibilities may include:
  • Provide skilled in-home speech-language pathology evaluations and treatment for adult and geriatric patients.
  • Evaluate and treat dysphagia, aphasia, dysarthria, cognitive-communication deficits, voice concerns, and related disorders.
  • Develop individualized, functional plans of care based on patient goals, clinical findings, and real-world needs 
  • Provide caregiver education and home-based strategy training.
  • Complete documentation in a timely, professional, and clinically appropriate manner.
  • Communicate with patients, caregivers, physicians, and care team members as needed.
  • Collaborate with Nexa leadership regarding clinical questions, patient appropriateness, scheduling needs, and caseload planning.
  • Maintain standards consistent with state licensure, payer requirements, and professional guidelines.

The ideal candidate is clinically curious, dependable, communicative, and comfortable working independently while still valuing team support.

  • Have a strong interest in adult medical or geriatric speech therapy.
  • Enjoy working with patients in functional, real-world environments.
  • Are interested in dysphagia, cognition, aphasia, dysarthria, dementia, Parkinson’s disease, stroke, TBI, voice, or progressive neurological conditions.
  • CCC-SLP or strong CF candidate with interest in adult medical care.
  • Want autonomy and clinical support.
  • Value flexibility but still take documentation, communication, and patient care seriously.
  • Are comfortable managing your own schedule and communicating proactively.
  • Communicate professionally and directly regarding availability, scheduling, documentation, compensation, and patient care.
  • Want to grow clinically in a supportive, clinician-owned practice.

Requirements:
  • Master’s degree in Speech-Language Pathology.
  • Active Florida SLP license, or eligibility for Florida licensure.
  • ASHA CCC-SLP preferred.
  • Strong CF candidates with relevant adult medical experience may be considered.
  • Professional liability insurance, or willingness to obtain.
  • Reliable transportation.
  • Ability to provide services in patients’ homes.
  • Strong documentation and communication skills.
  • Comfort with independent clinical decision-making and collaborative problem-solving.

Preferred Experience:
Preferred but not required:
  • Adult medical SLP experience.
  • Home health, outpatient, acute care, inpatient rehab, or SNF experience.
  • Dysphagia evaluation and treatment experience.
  • Experience with aphasia, dysarthria, cognition, dementia, Parkinson’s disease, stroke, TBI, or progressive neurological conditions.
  • Interest in FEES training and advanced dysphagia management.
  • Comfort communicating with physicians, caregivers, and interdisciplinary team members.

Compensation:
Nexa offers competitive per-visit contract compensation based on visit type, service area, clinician experience, availability, and current caseload needs.

This is a 1099 contract role. Compensation is not structured as a traditional hourly or salaried position, and any hourly amount displayed by the job board should be considered an estimate only.

To ensure clarity and consistency, compensation details are reviewed directly with each candidate during the initial conversation and are based on the specific role, visit type, service area, experience, availability, and current caseload needs.

Schedule:
This role offers flexible scheduling based on clinician availability and patient needs.

Clinicians may accept cases that align with their preferred service area, schedule, bandwidth, and clinical comfort. This makes the role especially appropriate for clinicians seeking supplemental work, flexible adult medical experience, or a more autonomous in-home care model.

Service Area:
Nexa is currently prioritizing clinicians available for in-home visits in Palm Beach County, with additional opportunities across  St. Lucie, Martin, and Indian River Counties. Additional South Florida service areas may be considered based on referral demand and clinician location.

Specific caseload geography will vary based on where the clinician lives, where patients are referred, and what is mutually sustainable.
